Satisfaction Survey
Community Ventures in Living is committed to being a responsive, client and family oriented agency that is driven by our core mission.
This requires developing performance measures that look at client and family satisfaction with CVL’s services and the manner in which our mission is implemented. One element of our performance measure is a Customer Satisfaction index. This index is developed, in part, from surveys collected from clients and their families who have used CVL services in the past 12 months.
We would like you to participate in this survey and provide feedback that we can use to improve our services. This survey is a series of 15 easy to answer questions that allow you to rate the services and interactions you have with our agency. There is also the opportunity for you tell us in your own words things that you would like to bring to our attention.
We know your time is valuable and we appreciate the time you take to give us this important information.
For the purpose of this survey "CVL Office Staff" represents staff that are indirectly supporting the community services such as the people in the office and scheduling staff. "In-Home Provider Staff" represents those that provide direct support to individuals in the community.
