Program Supports Coordinator - Evansville
We’re looking for our next Program Support Coordinator—someone who’s ready to help keep our programs organized, supported, and running at their best.
This full-time, benefited role supports the Evansville leadership team by coordinating services, keeping documentation accurate and up to date, and ensuring our programs meet the highest standards for individuals with developmental disabilities. Coordinates and supervises services and providers while acting as a communication link between families, providers, and the agency. Supports the Evansville leadership team by maintaining accurate documentation, ensuring program quality, and facilitating meaningful connections to individuals served, their families, and staff.
It’s a role that blends behind-the-scenes organization with meaningful connection to the people we support, their families, and our staff.
Supervisory responsibilities such as:
- Supervise and evaluate performance of providers.
- Provide management/support/directives to providers.
- Assist in the training and ongoing support of providers.
Other essential duties include:
- Monitor and update employee and contractor InFocus certifications.
- Complete new individual enrollment packets as assigned.
- Organize and complete tasks in given time frames.
- Communicate proactively with families regarding scheduled services and assigned providers.
- Conduct scheduled visits to homes of those involved in services.
- Provide necessary communication with agency personnel regarding individuals receiving services.
- Review end of month documentation submitted by Community Support Coordinators.
- Attend case conferences and required meetings as necessary.
- Attend annual meetings and update goals and risk plans as assigned.

Qualifications
- Associate’s Degree in Human Services/Socialwork or related field is preferred.
- 1 year Experience working with individuals with disabilities – required.
