Due to the Vaccine Emergency Temporary Standard through the Department of Labor which effects US employers who have 100 or greater employees, CVL is required to collect information on the vaccine status of all employees.  Please use the form below to provide your Covid Vaccine Status.  If you are vaccinated, please also upload a copy of your vaccination card.  If you have any questions regarding this request, please feel free to reach out through this form, email or phone call.
